Employee Withholding Forms & Direct Deposit Form - Each employee must complete the following documents, sign them, date them, and submit them to their employer so they can be sent to the company's payroll provider for direct deposit.

Form I-9
This is to verify the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must properly complete Form I-9 for every individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form.
W-9—Independent Contractor
All independent contractors must complete this form and return to you PRIOR to performing a service for your business. Provide the completed form to your payroll provider so they can issue a 1099.
Below are required forms for employees to be enrolled in direct deposit. If you are paying employees via physical check, only the W-4 and California DE-4 need to be filled out, along with the I--9.
Form I-9 is mandatory for every employee you hire. Follow all instructions very carefully. Form W-9 is mandatory for every independent contractor that performs a service for your business that will need a 1099 issued.
